What to expect on your first visit!

Upon entering our office, our staff will greet you with a smile. We will request that you complete our new patient forms. With this paperwork we will ask for your health history, other care you may have had, your job, and other questions designed to help determine the nature of your condition and the best way to go about treating the problem. We will then do a surface EMG scan and Dr. Jack will perform a spinal examination and analysis to detect any structural abnormalities which may be affecting or causing your condition. At that time Dr. Jack will determine if x-rays are necessary in order to make an accurate diagnosis and alert us of any serious spinal conditions. All of these elements are important components of your total health profile, and vital to evaluating your problem. Finally depending on your situation, Dr. Jack may give you an adjustment followed by electrical stimulation.

Report of Findings!

On your second visit Dr. Jack will sit down with you and discuss your diagnosis, surface EMG scan, x-rays, your treatment plan, and financial responsibility. We accept most health insurance; do not hesitate to call our office with any questions.
